Housebound people are some of the most vulnerable people in society, experiencing health inequalities and barriers to accessing health care.

The Home Health Team provides our PCN's housebound people with holistic and person-centred care for their long-term health conditions. Our vision is to improve the ease of access to routine health care for our housebound people to equal that of our non-housebound population.
