About Poplar and Limehouse Health Network
Serving the communities of Poplar and Limehouse
The Poplar and Limehouse Health and Wellbeing Network is a partnership of local general practices, community care teams, and local third-sector providers.
Our Vision and Aim
Our goal is to lead the way in creating a strong partnership between healthcare, social care services & community services in the Poplar and Limehouse area in order to:
Access and manage resources effectively
Create new and innovative ways to provide and deliver services that can reduce health inequalities
Make sure that patients and the public have better experiences with our services
We aim to enable patients to have independence, choice and control over their health care and provide integrated care at their first point of contact.
We aim to work collaboratively with our partners to ensure that we address our population’s health inequalities with low bureaucracy and mutual trust and respect.
Current projects:
The Network is involved in two pilots aimed at improving the health of patients suffering from long-term conditions:
A Children Asthma Pilot and
A Care Coordination Pilot
The Network alongside Public Health, is involved in a 3-year project aimed at enabling residents to overcome barriers to the prevention and early detection of long-term conditions. Through patients’ participation in prevention and detection initiatives, the project hopes to detect long-term conditions earlier, to prevent them and reduce health inequalities.
Legacy projects:
Communities Driving Change: in 2017 this programme was introduced as one of the main priorities of the Health and Wellbeing Strategy for Tower Hamlets. It ran for 3 years and adopted a community-based approach in which residents were empowered to take control of their health and wellbeing.
Go4Sport: this programme ran between 2014 and 2016 and provided patients who suffered from mental and physical health conditions the tools to get more active. It used sports on prescription to access one-to-one motivational support, Health Trainer weekly activity classes and sports on Sundays.
Other programmes delivered by the Network since its inception included a “Health Trainers Programme” and a “Your Move Programme” which both aimed to provide physical and mental health support to our residents.
